Sayyid Ibn Tawus and Shaykh al-Saduq have both narrated the following on the authority of Jabir ibn Abdullah al-Ansari: I visited the Messenger of Allah (Peace Be Upon Him and His Holy Household) on the last Friday of Ramadhan. As He saw me, He said, “Jabir, This is the last Friday of Ramazan. You should thus bid it farewell by saying the following dua. Verily, anyone who says this supplication will win one of the two good things; either attaining the next Ramadhan or having the forgiveness and mercy of Almighty Allah."
O Allah: (please) do not make it the last of our fasting in this month, but if You decide so, then (please) make me enjoy (Your) mercy and do not make me deprived (of it).
اَللّهُمَّ لا تَجْعَلْهُ اخِرَ الْعَهْدِ مِنْ صِيامِنا اِيّاهُ فَاِنْ جَعَلْتَهُ فَاجْعَلْنى مَرْحُوما وَلا تَجْعَلنى مَحْرُوما
This dua should be recited on the night before the last Day of Ramadhan. If it has not yet been confirmed when the last day of Ramadhan will be, then you may recite this on the night of the 29th and the night of the 30th of Ramadhan.
O Allah let not my days of fasting in (this) month of Ramazan be the last obligation (end of my life term). I take refuge with Thee lest the dawn of this night breaks forth without me having been forgiven.
اَللّهُمَّ لا تَجْعَلْهُ اخِرَ الْعَهْدِ مِنْ صِيامى لِشَهْرِ رَمَضانَ وَاَعُوذُ بِكَ اَنْ يَطْلُعَ فَجْرُ هذِهِ اللَّيْلَةِ اِلاّ وَقَدْ غَفَرْتَ لى
